I see we need x86_hyper_xen_hvm, I also see init_hvm_pv_info() uses
hypercall_page so that needs at least the hypercall page set up. That
means arch/x86/xen/xen-head.S needs some love in terms of enablement
when XEN_PVHVM is enabled but not XEN_PV or XEN_PVH, also enlighten.c,
time.c needs the XEN_PVHVM stuff split out and some common stuff set
out for both. That's what I'm up to at least.
