Please check ETVM (Eurotux Virtualization Manager). It's fully open-source and you can find information and download in http://etvm.eurotux.com.

 

We've just release 1.0.1 (bugfixes only). It has all the features that you request except for high availability (that should be in version 1.1 by June).
