For PVH version 2 you need Linux support and hypervisor support.  I
believe the earliest version of Linux that had PVHv2 support was 4.11.
The earliest version of Xen that had somewhat useable "tech preview"
PVHv2 support was probably 4.8.  The first version of Xen that
officially supported PVHv2 was 4.10; the XenProject Security Team have
backported full PVH support to Xen 4.8 via the "Comet" branch.
