[Xen-users] How to create PVHv2 DomU?

I'm not the first to notice, but to summarise: after the 4.9.0-5 kernel update, 
`pvh=1` in a Xen configuration file now causes Debian 9 DomUs to fail to boot.

According to the xen-devel list, this is intentional:

The above link states it's because `pvh=1` was based on the 'tech preview' of 
PVH, version 1.
Allegedly there is some kind of PVH version 2 that is coming into effect.

I can't find any information on how to actually use PVH version 2.

Does anyone know how to build an image, convert an image, or whatever, to 
actually use PVH version 2?
It seems quite critical given the latest vulnerabilities, but it seems unclear 
as to how to actually use it.
