I wasn't aware of mkstandalone at the time and I was mostly working with
Debian and basing my experiences on that.

It depends on the distro and the format of their ISO, not on Xen itself.
pvgrub2 etc just provides tools which distros can use to make this work.

For an installed distro things are "standard" enough that pvgrub2 will
normally just work, but distro installers have a lot more variation in
how they are setup etc and even on native using grub2 in this context is
fairly new (it's used for UEFI, things tended to use isolinux with old
BIOS).
