[XenPPC] patch status for noHV mode support

This is NOT for submission but to document the status of noHV mode work.
There are incomplete paths as noted in comments and missing function, such
as h_destroy_vm_area..  Two calls to h_vm_area_enter should be completed to
begin testing.
