You are correct, itlb_miss doesn't occur for xen code.
I think it is ok for putting code there.

Actually I'm using IA64_GRANULE_SHIFT, it can be configured to 16M or 64M.
Currently it's configured to 16M.
I don't know the best size for granule. :-)
