[Xen-ia64-devel] sync'd with xen-unstable

FYI, I just pulled xen-unstable into xen-ia64-unstable. This brings
in the new mmap'ing of the xsd_kva file in /proc and should make domU
with redhat work again. As Kouya reported, the root cause of this
problem may be something bad happening with freeing initrd memory, so
lets not forget the possible issue there even if the breakage is less
apparent. Thanks,

        Alex
