Actually, looking at it more, I think that's a red herring -
it's just that recent config changes changed the layout of the
kernel.

The real bug happened a while ago when reflect_interruption
was rearranged, and as a result the vcpu_force_data_miss path
(where a privop results in a TLB miss) no longer sets IHA.

(ITIR _was_ actually set, but in an location that doesn't
make much sense to the uninitiated, so I've moved that code
into vcpu_force_data_miss too.)
