Yes (though of course the entries are larger than 1 byte).  But Xen
only cares that it's at 0x10000000009 metaphysical; the virtual
mapping is up to the guest.

It's a normal metaphysical->physical translation.  It gets installed
by gnttab_setup_table using map_domain_page.
