Code for initializing/tearing down PMU for PV guests

Changes in v21:
* Use hypervisor-private VPMU context for PV(H) guests, just like we do for HVM
  guests. This means no special cases for PV(H) in arch-specific VPMU allocation
  and destruction routines. (i.e. patch changes to amd_vpmu_destroy(),
  svm_vpmu_initialise(), core2_vpmu_alloc_resource() and  core2_vpmu_destroy()).
* Replace 'ASSERT(mfn != 0)' with 'ASSERT(mfn_valid(mfn))' in pvpmu_finish()
