I would say it's certainly not common, but every OS with Dom0 support seems to do it (take a look at tools/include/xen-sys). Not sure if this was done in the past to allow compiling the toolstack on a host without a xen-classic kernel (which I guess was the only one to have the proper headers for user-space devices).
