It uses both "file:" and "phy:" and the former is AIUI blktap.
"file:" is used for the cdrom only, and only during HVM installs, so
the corresponding PV disk won't be heavily stressed.  If I'm not
mistaken with "file:" qemu-dm opens the file itself directly.
