HVM vcpu add/remove: parse 'vcpu_avail' to firmware logic and set up madt

- currently firmware has got 'vcpus' from xend, this patch add parse 
'vcpu_avail' to firmware;
- setup madt 'lapic' subitems of processors accoring to vcpu_avail;
