'[PATCH]' is not added automatically; people manually include the text '[PATCH]' at the beginning of the subject line for mails that include a patch, to draw attention to that fact. If you want to explicitly invite comments and feedback (such as on an idea or design for how to do something), you can also include [RFC] (request for comments).
