[Xen-bugs] [Bug 493] New: scsi0 (128:0): rejecting I/O to offline device

http://bugzilla.xensource.com/bugzilla/show_bug.cgi?id=493

Summary: scsi0 (128:0): rejecting I/O to offline device
           Product: Xen
           Version: unstable
          Platform: x86-64
        OS/Version: All
            Status: NEW
          Severity: normal
          Priority: P2
         Component: Hypervisor
        AssignedTo: xen-bugs@xxxxxxxxxxxxxxxxxxx
        ReportedBy: dbarrera@xxxxxxxxxx
                CC: hohnbaum@xxxxxxxxxx,pl@xxxxxxxxxx

Running 11 domUs on Xen unstable, changeset 8671, SLES 9 SP2 based system, IBM
xSeries 460, with 8GB of RAM. The system has two SCSI drives, sda and sdb
(SAS/SATA) drives that use the AIC94xx driver. I seem to have lost the sdb
drive. I have several domUs who reside on the sdb drive and are erroring as
follows:

x460-xentest-vm11 login: root
end_request: I/O error, dev sdb15, sector 11404024

Dmesg on dom0 shows hundreds of messages indicating "rejecting I/O to offline
device", which is sdb.

dmesg output on dom0:
