Hi,

this patch removes VHPT_CCHAIN_LOOKUP and inlines its body in ivt.S
vhpt_insert is now written in C (shorter!)

Now ivt tlb_miss entries could be debugger more easily and should be cleaned.

Tested by boot+halt of dom0+domU.
